Peili Vision wants to test children for ADHD by watching how they cope with everyday tasks in virtual reality. The Finnish company built on research from Aalto University a test in which children instead do everyday errands in a virtual apartment while the software measures how they plan, remember and stay on task. It now sells it as EFSim.
The European Innovation Council granted €2,496,112 towards VR4NEURO, a €3.57 million project that ran from November 2022 to March 2025 under the title "Building the world's largest digital hospital for neurological challenges". According to the CORDIS report, the diagnostic tool reached "up to 98% sensitivity in ADHD detection with an AUC of 88%" and won CE Class IIa certification. In July 2024, however, England's health technology assessor NICE concluded that the evidence for the EFSim Test was lacking and that it should be used only in research, pharmaphorum reported. Peili Vision's revenue was €19,000 in 2025.

Where Peili Vision Stands Today
The company's old website now redirects to efsimtest.com, which markets EFSim to healthcare professionals and schools. It publishes no customers, prices or usage figures. The Finnish accounts summarised by Asiakastieto show revenue falling from €105,000 in 2023 to €66,000 in 2024 and €19,000 in 2025, and staff falling from 19 to 10. We found no funding round announced after the EIC grant.
The Technology: Errands in a Virtual Flat
The research game, EPELI, was tested on 112 children aged 9 to 12, 47 of them with ADHD, as Aalto University reported in January 2022. Its lead researcher said its accuracy was "already comparable to that of CPT", the standard computerised attention test. VR4NEURO added a clinician portal, AI-adapted rehabilitation exercises for executive function, and links to electronic patient records, tested in the sandbox of Finland's national Kanta system.

The project's title promised "the world's largest digital hospital for neurological challenges". The report describes a diagnostic tool, a rehabilitation system, a clinical portal and a care-pathway manager, with CE Class IIa certification. It names no hospital or health system using the platform, and its next steps are further studies in autism, pilots in other health systems and FDA approval.

What Has Happened Since the Award
The Money
Peili Vision's operating losses have been modest, €168,000 to €402,000 a year between 2023 and 2025, and its equity ratio was 20% at the end of 2025. With revenue falling and staff halved, the accounts show a company shrinking after the grant ended.
What cannot be checked
How many clinics or schools use EFSim, whether any health system pays for it, and whether the company has sought a new review by NICE are not public.
Why Europe Should Care
The project's own aim was to cut diagnostic bottlenecks with a digital test. The EIC paid for certification and product development. The independent assessment that matters for health systems, NICE's, found the evidence insufficient, and the company has not grown.
