Hephaistos-Pharma, founded in Orsay near Paris in 2018 and led by chief executive and co-founder Frédéric Caroff, develops an immunostimulant that activates the TLR4 receptor of the innate immune system and, unlike existing TLR4 drugs, can be given intravenously, so that it could turn "cold" tumours that ignore immunotherapy into "hot" ones throughout the body, including metastases. It was selected in the EIC Accelerator's June 2023 cut-off with blended finance and received an EU grant of €2.50 million for IVI-Boost, a €6.91 million project running from May 2024 to January 2027 to industrialise manufacturing, win clinical trial authorisation and validate "clinical safety and efficacy in a phase 1/2 clinical trials", according to CORDIS. The company aimed "to become a clinical stage company by 2026".

The first report, to March 2025, says Hephaistos created a Belgian subsidiary for industrialisation, adapted its process to GMP conditions, transferred production to a contract manufacturer with transfer runs starting in February 2025, and had its toxicity study protocol validated with the French medicines agency ANSM, according to the CORDIS reporting page. It says Hephaistos launched a €20 million Series A "with the support of historical VCs for 4M€ and a 5M€ ticket from the EIC fund", aiming to close "by the end of 2025". No closing has been announced. In May 2026 the company named Stephan Braun chief medical officer to prepare the clinical development of HEPHA440, "a 3rd generation TLR4 agonist small molecule", toward first-in-human studies, according to Hephaistos-Pharma.

Where Hephaistos-Pharma Stands Now

Hephaistos completed a €4.5 million seed round led by Elaia in May 2024, alongside €5.8 million of non-dilutive funding from the EIC, Bpifrance and France 2030, according to Hephaistos-Pharma. It reported no revenue and a net loss of €361,894 in 2024, according to the French company directory. The last changes to its share capital were registered in September 2024, according to BODACC. Its 2026 announcement lists a new chief development officer and head of manufacturing operations alongside the chief medical officer. It does not say how HEPHA440 relates to ONCO-Boost, the product named in its EIC report.

The Technology: Waking the Immune System by Drip

Immunotherapies fail in many cancers because the tumours attract few immune cells. Drugs that stimulate the TLR4 receptor can fix that, but those in use today must be injected into the tumour because they are too toxic in the bloodstream, which rules out most metastatic patients. Hephaistos says its molecule is the first TLR4 modulator that can be safely given intravenously. Its evidence so far is preclinical, in models of osteosarcoma, colorectal cancer, lymphoma and breast cancer.

What the EIC Award Promises, and What the Record Shows

The project has three milestones: industrial manufacturing, clinical trial authorisation and a Phase 1/2 trial. The first is under way. No trial authorisation or trial registration has been announced, and with the project ending in January 2027 the Phase 1/2 results it promised cannot arrive in time. The funding that would pay for the trial, the Series A including the EIC Fund's €5 million, has not been confirmed.

The Money

The EU grant covers 36% of the project; the company must fund €4.41 million itself. In 2022 Hephaistos expected a first clinical study by 2023, according to its orphan drug announcement. That date has moved by at least three years.

Whether the Series A closed and the EIC Fund invested, whether HEPHA440 is ONCO-Boost or a new molecule, whether a trial application has been filed, and whether the project will be extended are not public.

Why Europe Should Care

A researcher at Lyon's Centre Léon Bérard, one of the company's partners, has said there has been "no therapeutic progress against osteosarcoma for 40 years". A safe intravenous immune booster could help them and many adults with metastatic cancer. Hephaistos has the preclinical data and a manufacturing route; it has not yet shown the drug in a single patient.
