contextflow, founded in Vienna in 2016 as a spin-off of the Medical University of Vienna and the EU research project Khresmoi, develops AI software that helps radiologists detect and track lung cancer, interstitial lung disease and COPD on chest CT scans. It was selected in the EIC Accelerator's March 2023 cut-off for "Integrated AI-based Clinical Decision Support for Radiologists" with blended finance, and its website says it received "both grant and investment awards from the European Innovation Council". No project for contextflow appears on CORDIS, the EU's register of funded projects, so the amounts and results are not public.
On 1 June 2026 4DMedical, a lung imaging company listed on the Australian Securities Exchange, announced a binding agreement to buy contextflow, and contextflow says the exit was completed in August 2026. 4DMedical paid about A$18.56 million (€11.4 million) in cash upfront plus 56,235 shares and an earnout of up to 2.59 million share options, and described the terms as significantly discounted; it also retains about €19 million of accumulated tax losses, according to Trending Topics.

Where contextflow Stands Now
contextflow is now part of 4DMedical, which said the deal gives it an immediate European commercial and clinical presence with an active team, customer base and CE-certified product portfolio. Trending Topics described the sale as "more pragmatic than triumphant", given the discounted terms and the large tax losses, despite contextflow's reimbursement breakthrough in Germany. In November 2024 contextflow announced it was set to secure what it called Europe's first radiology AI reimbursement contract, with IKK Südwest, according to its announcement.
"As part of the acquisition, 4DMedical retains a cumulative tax loss of €19.0 million (approx. A$30.8 million), which may potentially be used to offset future taxable income from the Austrian business and represents a significant additional source of value."

contextflow's software analyses chest CT scans to find and measure lung nodules, assess malignancy and flag incidental findings such as pulmonary embolism, and can retrieve visually similar reference cases. Its products are CE-marked under the EU's Medical Device Regulation and in routine clinical use, according to the company and 4DMedical.
What the EIC Award Promised, and What the Record Shows
The award was to scale integrated AI decision support for radiologists. Without a CORDIS project, there is no public record of what the EIC paid, what milestones were set or whether the EIC Fund invested. The company did achieve a German reimbursement contract and partnerships with hospitals and imaging platforms, but it was sold three years later for a price the buyer called discounted.

The Money
contextflow raised a €6.7 million Series A in 2021, according to Tech.eu, and several million euros in grants, Trending Topics reports. The €19 million of tax losses transferred to 4DMedical is a measure of the losses the company accumulated. What the EIC Fund, if it invested, received from the sale is not public.
What Cannot Be Checked
Questions for the EIC
How much the EIC paid as grant and equity, why no project record exists, and what the EIC Fund recovered in the sale to 4DMedical are not public.
Why Europe Should Care
contextflow was a European leader in radiology AI with EU roots going back to the Khresmoi research project. Its technology and team now belong to an Australian listed company, which bought it to enter the European market. EIC support helped build an asset that a non-European buyer acquired on discounted terms.
What the award promised
Scaling AI-based clinical decision support for radiologists reading chest CT scans.
What the record shows
A certified product with a German reimbursement deal, then a discounted sale to an Australian company.
The Verdict
A European asset, sold abroad
contextflow built a working product with EU support at several stages. It ended as a discounted acquisition by 4DMedical, and the EIC's return is unknown.
